Dean's Professional Services is now hiring a qualified Care Coordinator II to work for a HealthCare Organization in Texas . - Remote Position must reside in Texas

The Care Coordinator II is responsible for supporting care management activities and the teams assigned to members to ensure services are delivered by the healthcare providers and partners and continuity of care/member satisfaction is achieved. Interacts with members by performing member outreach telephonically or through home visits and documents the plan for care/services of activities.
Job Duties :
* Manage the process of getting DME, medical supplies, and adaptive aids for members.
* Inform members about their DME benefits and what they need to do to get the items they require.
* Assist members in scheduling appointments with their primary care physicians when needed for DME assessments.
* Find DME agencies that can provide the necessary equipment and work with them to place orders.
* Work with PCPs, DME agencies, and members to make sure all requirements are met for authorizations and DME orders.
* Ensure members receive the DME items they need and help solve any problems they face.
* Check with members after delivery to ensure they are satisfied with the DME items.
* Keep accurate records of all DME-related activities, including communications, orders, and follow-ups.
* Ensure all actions follow Waiver rules and processes.
* Collaborate with case managers, healthcare providers, and others to ensure smooth coordination of services.
* Take part in training and offer suggestions to improve DME services based on member feedback.
* Performs other duties as assigned
* Complies with all policies and standards
Job Requirements:
* Remote Position must reside in Texas
* High School Diploma or equivalent - Required
* 1 year of experience; thorough knowledge of customer service, experience in a managed care environment or healthcare field. - Required
* Experience operating office equipment/computer - Required
* Knowledge of medical terminology - Required
* Experience in DME or Healthcare setting - Preferred
* Previous Work From Home Experience - Preferred
Job Details:
* Schedule: Full time-M- F 8 am - 5 pm
* Pay-$17/hour + benefits.
