POSITION SUMMARY
The Court Liaison is responsible to monitor individuals that are under Chapter 51.20 or Settlement Agreements. Court Liaisons facilitate linkages to community resources to comply with treatment recommendations and court orders.
RESPONSIBILITIES
1. Chapter 51
a. Provide linkage and follow up services to patients that are under Chapter 51.20 or Settlement Agreement.
Communicate with identified providers regarding the Chapter 51 or Settlement Agreement. Responsible for providing copies of commitment order or settlement agreement to mental health prescriber.
b. Notify treatment providers of client's noncompliance with Settlement Agreement. Monitor and report to Corporation Counsel all clients who are in noncompliance of court ordered treatment plan under 51.20 commitments.
c. Review commitments and prepare quarterly report to support recommendation regarding extension of commitment order to the Court and Corporation Counsel within statutory time limits.
d. Complete or update a crisis plan on all Settlement Agreement and Commitment Orders within 30 days of hospital discharge. Revise the crisis plan every 6 months.
e. Provide face to face or phone appointment prior to the client's discharge from inpatient care.
f. Provide weekly face to face (or the like) with clients residing in Kenosha County for the first 30 days discharged from hospitalization.
g. Provide weekly phone staffing with client and/or support staff residing outside of Kenosha County.
h. Provide a face to face (or the like) visit within 2 business days of client's admission to KARE Center. Assist in the discharge plans. Document visits in both CTS and KARE Center blue notes.
2. Liaison for hospital and/or group home replacement
a. Attend court hearings related to Chapter 51 and probable cause hearing.
b. Inform client of court liaison's role, legal process, and statutory time limit utilizing the post discharge checklist.
c. Maintain and monitor internal file containing client's legal information.
d. Coordinate with hospital/group home staff and treating psychiatrist to formulate appropriate discharge plans for clients who are under a legal hold.
e. Act as a liaison with client's treatment team, including family members, in the planning and possible placement into residential or state facility.
f. Act as a backup to other team members in the division.
g. Attend peer review, weekly team meeting, weekly IMD residential review, and Chapter 51 committee meeting.
3. Reporting
a. Complete all documentation related to new cases to include but not limited to: functional screenings, referrals to case management, post discharge checklist, crisis plan, release of information, and provider letters.
b. Document all contacts in SharePoint and CTS within 5 business days.
QUALIFICATIONS: Requires a minimum of a Bachelor's Degree from an accredited college or university. Master's Degree preferred.

2. Must have reliable transportation and valid driver’s license with good driving record
3. Must carry own auto insurance with at least 100,000/300,000/100,000 coverage
4. Ability to maintain records and document case notes daily
5. Ability to give and accept feedback as a part of a team
6. Ability to traverse within the workspace and community frequently
7. Be willing to learn and work with the treatment philosophies
8. Bilingual in Spanish a plus
