We're seeking a dedicated Home Care Nurse Coordinator to join our growing team.
What You'll Do:
As a Home Care Nurse Coordinator, you'll be at the heart of our client care, coordinating and overseeing healthcare services for individuals in the Lakewood, Littleton, and Denver metro areas. You'll collaborate with healthcare providers, assess client needs, develop personalized care plans, and provide guidance and support to our dedicated team of caregivers. You'll play a vital role in ensuring our clients receive the highest quality of care and support they need to thrive.
Responsibilities:
Coordinate and manage healthcare services for clients.
Collaborate with healthcare providers to ensure quality care.
Assess client needs and develop individualized care plans.
Provide guidance and support to healthcare staff.
Ensure compliance with healthcare regulations and standards.
Communicate effectively with the team and clients.
Maintain accurate medical documentation.
Conduct annual in-person supervisory visits.
Perform record audits.
Provide education to clients, employees, and families.
Utilize our care management software program.
What You'll Bring:
1 year of experience practicing as an RN (required), with additional experience in acute care highly preferred.
Med/surg experience is a big plus!
Familiarity with the disabled community is preferred.
1 year of supervisory experience overseeing caregivers and other care providers.
Spanish speaking skills are highly valued.
Current, unencumbered RN license in Colorado.
BSN is highly preferred.
Current CPR certification is required; First Aid certification is preferred.

Location: Primary caseloads are available in Westminster, Broomfield, and Lakewood, with some clients in Longmont, Loveland, and Boulder, or in Aurora, Centennial, and Littleton.
Work Environment: Hybrid - a mix of remote work and in the field, with Tuesdays and Thursdays in the office.
