Your Role:
As an Occupational Therapist (OT) in home health, you will work alongside the Case Manager to provide essential services aimed at improving patients' ability to live safely and independently in their own homes. You will evaluate and address needs related to home safety, medication management, wheelchair seating, pain management, fall prevention, chronic disease management, dementia care, and caregiver training. You will also serve as an educator, advocate for your patients, and collaborate as part of a multidisciplinary care team.

What We're Looking For:
Qualifications:
Current unrestricted CA Occupational Therapist License
BLS/CPR Certification
Valid CA Driver's License
Knowledge of local community resources
Experience in home health care is preferred
Experience with high-acuity patients is a plus
Familiarity with interdisciplinary teams is highly desired
Knowledge of state and federal homecare guidelines is a plus
Case Management experience is a plus

Compensation:
Salary Range: $120,000 - $130,000 per year, depending on experience
Additional Benefits: Generous bonus incentive plans, comprehensive health benefits, and 401k (up to 4% match)
